Before starting to fill the application, please prepare a digital photocopy of your personal identity card or driving license or passport. It must be a document with a photo that officially identifies you. Prepare also a copy of utility bill that contains your name and address matching identity document(s). You will have to upload them at the end of this application form. The image files should be preferably in JPG/JPEG or PNG format. The size of image data shall be less than 2 Megabytes. Please, avoid other file formats, sending larger file or images embedded within document files.
